Looking for helper for extremely large person- cooking, walking, etc.

I'm looking for a service or a person to help me a few times a week with things like helping me go on walks, shopping, cooking, random things I need help with, etc. I'm a 38 year old guy and I have very limited mobility due to my size.
A few times a week, I would like someone who can help me get out of the house and go for a short walk, cook me some meals, grocery shopping for food and snacks, run some errands with me, etc.
